The Michigan Department of Health and Human Services (MDHHS) Home Help Program is designed to help seniors and individuals with disabilities receive the support they need to remain safely in their own homes. The program provides financial assistance for approved in-home services when a person is unable to perform certain daily activities independently.
Step 1: Check Basic Eligibility
To qualify for the MDHHS Home Help Program, the individual must live in Michigan and have a physical, cognitive, or medical condition that limits their ability to perform daily tasks. The person must also meet the program’s financial eligibility requirements.
Step 2: Complete the Application
Applicants must complete the required MDHHS forms, including the DHS-54A and DHS-390 forms. These forms provide information about the applicant’s personal situation, health condition, and the type of assistance needed.
Step 3: MDHHS Assessment
After the application is submitted, an Adult Services Worker (ASW) from MDHHS will conduct an assessment. During this process, the ASW evaluates the individual’s ability to perform Activities of Daily Living (ADLs) such as bathing, dressing, mobility, and eating.
Step 4: Service Approval
If the applicant qualifies, MDHHS will approve specific services and determine the number of hours of care authorized. These services must follow the program’s “Time and Task” guidelines.
Step 5: Begin Home Care Services
Once approved, care services can begin through a qualified caregiver or home care agency. Services typically include personal care assistance, meal preparation, and light housekeeping.
